Nurses moved from discomfort with reflective practice toward greater confidence and insight. Low uptake of peer coaching across implementation sites was described as surprising and concerning. Peer coaches used reflective diaries for individual and group coaching. Kalorama Reflective Coaching supports nurses in reflecting on person-centred attitudes and behaviours. Reflective diaries connected abstract autonomy principles to concrete everyday care encounters. Nurses used reflective diaries to record real care situations involving self-direction and possible alternative actions. Enabling peer coaches to access and record consultations on primary care EPR systems was a major technical and administrative barrier. Peer coaching was offered at only one of four implementation sites throughout the study. An approved, mandatory EPR access solution was identified as necessary for the peer coaching element to function.
