Abatacept has shown preventive or delaying effects in other high-risk prearthritis populations. In ARIAA, rheumatoid arthritis was diagnosed less often with abatacept than placebo among patients with arthralgia, ACPA positivity, and MRI subclinical inflammation. In APIPPRA, abatacept lengthened time to rheumatoid arthritis compared with placebo. Longer abatacept treatment may further reduce progression risk, but the PALABA data do not prove this. The prevention concept involves identifying higher-risk people before persistent rheumatoid arthritis and intervening during a window of opportunity.
