SAFER-Dem aims to address communication gaps and help people with dementia and unpaid carers feel heard and informed during discharge. Stakeholders viewed SAFER-Dem as a way to structure conversations and support shared decision-making in discharge planning. SAFER-Dem was refined into a final prototype for this co-design stage but still requires wider feasibility testing before implementation. SAFER-Dem adapts the SAFER-Mental Health discharge care bundle for people living with dementia leaving UK mental health inpatient settings. The project used stakeholder insights to co-design and refine discharge planning materials addressing barriers in discharge processes.
