Semaglutide (Wegovy) achieves approximately 10–15% weight loss for weight management. Semaglutide reduces the risk of developing Alzheimer's disease more significantly than other GLP-1 receptor agonists. Oral semaglutide (Rybelsus) is the only oral GLP-1 receptor agonist available. Although liraglutide and semaglutide share structural similarities, they exhibit different brain distributions in rodents, suggesting molecule-specific differences in brain penetration. Semaglutide exhibits stronger peripheral anti-inflammatory effects than placebo or exenatide in T2DM patients. In the MOST-ABLE trial, all semaglutide arms are administered once daily as add-on to conventional PD medications, and the drug must be discontinued rather than dose-reduced if intolerable. Semaglutide met the steatohepatitis resolution endpoint in T2DM patients, but fibrosis improvement did not reach statistical significance versus placebo.
