Earlier SICP models commonly included formal clinician training and systematic distribution of preparatory and family materials, unlike this pilot. The programme was implemented as a system-level intervention to help clinicians identify suitable patients, initiate serious illness conversations and document outcomes. The SICP was originally designed to help clinicians conduct earlier and better serious illness conversations through communication tools and system supports. The study evaluated whether a coordinator-facilitated SICP could be adapted and implemented in outpatient oncology, cardiology and palliative medicine clinics in Singapore.
