A just-in-time simulation format, where bronchoscopists perform a perfect simulated bronchoscopy immediately before each clinical procedure, could substantially increase skill transfer. Virtual reality simulators and bronchoscopy phantoms have been shown effective for simulation-based training and are potentially superior to traditional clinical apprenticeship. Simulation-based training with mastery learning has already proven superior to traditional clinical apprenticeship in EBUS bronchoscopy and surgical procedures such as hernia repair. The mastery-learning AI simulation training intervention did not produce statistically significant improvements in any primary or secondary outcome measure in intermediate or experienced bronchoscopists. Only four studies prior to this one had measured whether simulation skills actually transfer to improved real-world clinical performance. Novice bronchoscopists have higher complication rates, cause greater patient discomfort, and yield fewer positive biopsy samples than experienced practitioners.
