Sphincter of Oddi syndrome is mechanistically the same problem as other bile obstructions — obstruction by sludge or stone at a critical drainage point. The sphincter of Oddi is a valve at the point where the bile duct and pancreatic duct meet and empty into the small intestine. When sludge or micro-stones obstruct the sphincter of Oddi, pressure backs up into both the bile duct and pancreatic duct.
