When the puborectalis relaxes through squatting, bowel contents can be released with ease. Knee elevation significantly reduces the physical pressure required during bowel movements. The heels should be positioned roughly 8 to 12 inches from the wall. Squatting remains the natural bowel movement position used in many countries worldwide. Bending the knees slightly and gently tucking the hips flattens the lower back against the wall and creates a supported squat-like position. Foot placement is estimated using the wall by placing one foot against it and using the big-toe position to set heel distance. Maintaining the squat stance and avoiding loss of wall contact are key positional cues throughout the drill.
