Observed hospital mortality increased across SMR categories in the study population. The SMR is calculated as observed hospital deaths divided by expected hospital deaths for an ICU in a year. For each ICU-year, expected deaths were calculated by summing individual APACHE-IV predicted mortality risks. Expected hospital deaths are based on risk-adjusted mortality predictions, especially the APACHE-IV model.
