State-Aware Dialogue Framework

The internal uncertainty estimate guides questioning but is not a calibrated diagnostic probability and is not shown to users. The framework organizes the consultation into history-taking, diagnosis and management, and follow-up phases. A structured patient profile is initialized and updated throughout the conversation. The framework uses intermediate outputs for patient state, hypotheses, uncertainty, and knowledge gaps. After validation, the system presents a ranked differential diagnosis of 5 to 10 conditions grounded in the full interaction and multimodal findings.
