A sustainable, high-quality model should preserve the well-being of students, clinical mentors and patients while developing student competencies. Organizational sustainment is defined as continued delivery or institutionalization for more than twelve months after implementation, with capacity for adaptation. Individual maintenance is defined as physical activity behavior change lasting more than six months after the intervention ends. The protocol defines sustainability as maintaining the practicum model over time without compromising human, organisational or economic resources. The service effectiveness evaluation cannot establish definitive causality because it lacks a randomized control group. Individual-level maintenance will be assessed at 6 months using healthcare use, repeat helpline use, and health confidence. Cost-effectiveness will be evaluated from a health-system perspective using quality-adjusted life-years. Service-level sustainability will be examined through qualitative interviews, stakeholder focus groups, administrative data, and cost-effectiveness analysis.
