Tirzepatide (Zepbound) achieves approximately 12–20% weight loss when used for weight management in adults. Tirzepatide (Mounjaro) achieves a 1.7–2.3% reduction in A1C for type 2 diabetes. Tirzepatide is the first approved dual GIP/GLP-1 receptor agonist. In this trial, tirzepatide starts at 2.5 mg weekly and can be escalated to a maximum maintenance dose of 15 mg per week. Tirzepatide binds both the GLP-1 receptor and the glucose-dependent insulinotropic polypeptide receptor. Tirzepatide commonly causes gastrointestinal adverse effects, which are usually mild and can be reduced by gradual titration. Patients who discontinue tirzepatide after a run-in period regain substantial weight compared to those who continue treatment. Tirzepatide is being evaluated as an obesity treatment for adults with type 1 diabetes rather than as a direct glycaemic-control therapy. Tirzepatide is not approved for glycaemic control in type 1 diabetes because a major direct glucose-lowering effect is not expected in this group.
