Tocilizumab

Preliminary non-peer-reviewed data from one hospital system reported 75–80% survival in mechanically ventilated patients treated with the combination of steroids plus tocilizumab. Whether combining dexamethasone and tocilizumab produces additive benefit without additive immunosuppressive risk requires formal RCT confirmation. Clinical evidence for tocilizumab in COVID-19 was emerging but not yet definitive at the time this session was recorded. Tocilizumab is a monoclonal antibody that specifically inhibits the IL-6 receptor, targeting one key driver of cytokine storm rather than broadly suppressing all immune activity. Tocilizumab inhibits interleukin-6 by binding the cytokine and preventing it from reaching its receptor, thereby blocking the downstream inflammatory cascade driving cytokine storm. Tocilizumab was reserved for patients with confirmed bilateral COVID-19 pneumonia who were deteriorating despite other treatments.
