The most common definite-group ulcer sites were the sacrum, heel, and coccyx. D2 was the most common depth in all groups and was highest in the definite group. The DESIGN-R2020 scale includes seven components: depth, exudate, size, inflammation or infection, granulation tissue, necrotic tissue, and pocket. Wound severity was evaluated using the DESIGN-R2020 scale. The definite group had 158 pressure ulcers across 128 patients, indicating some patients had multiple ulcers.
